An initial payment of $297 and monthly payments of only $99 will get you the following benefits:

Terms and Conditions:

  • This is a 12-month membership and auto payments automatically renew unless the patient notifies RejuveMD Med Spa to not renew at 12-months.
  • You will be required to sign our Membership Agreement at your first visit.
  • Cancellation must be presented in writing or in person with a 30-day notice. A $99 cancellation fee will apply unless the term completed.
  • Cancellation will be allowed without penalty if you are unable to utilize the membership due to a disability, illness, injury, change in residence greater than 45 miles from RejuveMD Med Spa, or you are being called to active duty by the United States Military and deployed outside of the state of Texas. In the event of cancellation requests for these reasons, a copy of the medical doctor’s note, or proof of relocation, or official active duty orders must be provided with written cancellation.
  • Once cancellation notice is received by RejuveMD Med Spa with patient/member intent to discontinue membership, no further services will be rendered.
  • Any/all previously collected membership payments are non-refundable.
  • All included services rendered in-office.
  • Appointment required.
  • Services must be used in the month in which they are earned or will be forfeited.
  • Upgrades may be added at the time of service for an additional fee.
  • Additional Dysport units will be available at the time of service for a discounted rate.
  • We reserve the right to refuse membership to anyone at our discretion.
  • We reserve the right to change the terms of this membership at any time without notice.
  • Member discounts are not combinable with other promotional discounts.
